Job Summary
Under general supervision, performs inpatient nutrition re-screening, re-assessment and basic diet education on low nutrition risk patients. Utilizes computer system to process menus and diet information. Performs routine clerical and tray line duties.
Essential Functions:
Performs screening and completes within 72 hours of assessed need, referral, risk notification, or MD consult. Performs re-screening and re-assessments and completes according to level of nutrition risk time frames. Performs nutrition re-screening and re-assessment for pediatric, adolescent, adult and geriatric age groups under the direction of the dietitian. Exercises knowledge of age-specific and disease specific nutrition standards of care including appropriate interventions/Medical Nutrition Therapy and applies to patient care activities. Utilizes methods, techniques and approaches to the delivery of care that are consistent with the age group of the patient and in conformance with the specific care plan developed for each patient. (35%)
Provides education demonstrating respect to patient/family?s education level, optimal learning tool(s)/technique(s), and readiness to learn. Provides appropriate nutrition counseling and documentation within 48 hours of assessed need. Provides patient education according to policy and in line with Food/Drug Interaction practice. Performs menu and patient care activities. Adjusts and corrects menus according to diet order and patient preferences. (35%)
Clarifies incomplete or ambiguous diet order efficiently. Initiates discussion for need of supplement/nourishment with patient and refers to MD/RD to place order for supplement/nourishment in electronic medical record. Performs intake analysis (calorie counts) as assigned. Effectively uses computer software/electronic medical record to perform diet office functions and clinical activities. Performs tray line duties. (15%)
Assists with and/or provides coverage for diet office/diet clerk duties as needed e.g. answer phones, computer data entry, filing, etc. Maintains clean and organized work area. Performs PI/CQI projects as assigned by supervisor. Serves as a resource to the department and staff by conducting in-services on information obtained from educational meetings/in-services. Obtains patient food preferences and effectively handles patient/customer requests and complaints. Discusses patient concerns/problems with the dietitian effectively and timely. Documents patient care activities on appropriate forms and according to accepted hospital practice. (15%)
Education:
Associate's degree in nutrition, dietetics, or directly related field required
Work Experience:
1-3 years experience with food service operations required
Licenses:
Dietetic Tech, Commission on Dietetic Registration required
Knowledge, Skills and Abilities:
Effective communication skills in both oral and written form.
Ability to exercise good independent judgment.
Ability to follow written and oral instructions. Process the ability to assess data reflecting the patient?s status and interpreting appropriate information.
Demonstrates knowledge of disease states and medical terminology
Demonstrates understanding of Local, State, Federal and The Joint Commission standards and regulations.
Demonstrates competency with computer system for order entry and to update diet and patient information.
Demonstrates understanding of how to use most current version of on-line Nutrition Care Manual
