This position provides high quality, compassionate healthcare promoting health and wellness in our community. The diabetes clinician works directly with patients and their families to provide individual and group education and support to people with diabetes and other chronic health conditions. Utilizes standards, guidelines and best practice for care delivery. Incorporates data and information to continuously improve care and practice to enhance outcomes. Communicates effectively and works cooperatively with others. Has respect for and understanding of other disciplines. Uses an integrated approach toward patient outcomes. Provides clinical services and acts as a resource to physicians, nurses and other professional health care team members to provide patients with optimum nutrition care. Serves as a resource, preceptor and mentor. Ensures improvements in practice settings by assuming responsibility for self-development in life-long learning. Demonstrates leadership skills in decision-making and problem solving.
Preferred / Desired
Two years experience in diabetes patient care management and education.
Eligible for certification as Certified Diabetes Educator
AL Dietitian & BLS within 90 days of hire
