The dietitian will perform clinical nutrition therapy, outpatient diet education, and community education for the Graham Health System patient population. Job duties include, but are not limited to, the following:
Monitors nutrition screening on newly admitted patients to identify those at nutrition risk.
Performs thorough nutrition assessments using the Nutrition Care Process. Identifies areas for nutrition intervention and provides accurate documentation. Communicates any recommendations to physician and staff.
Provides follow-up nutrition assessments as needed and per policy
Analyzes and documents calorie counts.
Provides nutrition support assessments and recommendations.
Offers diet education for all patients with an admitting diagnosis of CHF and Diabetes. Perform other diet education per physician consult request.
Performs medical nutrition therapy (MNT) sessions or nutrition counseling/education for all outpatient referrals per CMS guidelines and according to the Scope of Practice.
Attends formal Pritikin Intensive Cardiac Rehab (ICR) training, and maintains appropriate standing in regards to the knowledge and understanding of the content contained in the Pritikin program material, including:
Performing one on one Registered Dietitian counseling visits with each new phase II, Pritikin Intensive Cardiac Rehab patient.
Setting applicable goals with each patient, and follows up on those goals throughout their duration of the program.
Leading Pritikin, nutrition specific educational workshops/classes, according to the Cardiac Rehab class schedule.
Coordinating and leading weekly Pritikin cooking schools (one day per week), in which education is given to the patients, and the food items are prepared during the class.
Bachelor's degree in nutrition or health education
Registered Dietitian in the State of Illinois
Licensed Dietitian in the Sate of Illinois
Food Handler's Training and Certification (must be obtained within 30 days of hire)
2 years clinical and/or outpatient education experience preferred
CMS provider number required
