Under direction of the Chief Support Officer (CSO), the Certified Diabetes Educator educates patients on the necessary lifestyle changes involved in diabetes diagnosis and management.
Serves as Quality Coordinator providing oversight of Diabetes Self-Management Education and Support (DSMES) services, ensuring:
Implementation of the standards
Services are evidence-based
Service design incorporates population needs
Ongoing service evaluation and continuous quality improvement plan is reviewed at least annually
Collection and evaluating of data to identify gaps in services
Outreach to key stakeholders for their input on services, quality, utilization, access, and sustainability
Educates patients on the necessary lifestyle changes involved in diabetes diagnosis and management, including nutrition, medication, physical activity and glucose testing. Proactively manages the patient's condition.
Performs assessments and data collection; establishes outcome criteria; obtains, reviews, and analyzes information in collaboration with patients, provider, and care team.
Facilitate diabetes and nutrition education classes, while tracking demographic data and quality outcomes.
Provides community education at outreach events.
Provides in-service training to HMS departments and clinical staff.
Maintains a library of accurate and appropriate education materials as an adjunct to counseling and education.
Other duties as assigned by supervisor.
Certified Diabetes Educator credentialing
Three years of clinical experience
Experience facilitating behavior change
Current CPR Certification
Skilled in the use of electronic health records
NM licensure as Registered Dietitian or a Registered Nurse
Knowledge of nutritional sciences and current treatments for diabetes and related manifestations
Experience giving presentations and facilitating group classes
Effective verbal and written communication skills; written skills in English required
Ability to work both independently and in a team environment
